Nahrungsfasern vermögen sowohl bei Nicht-Diabetikern als auch bei Typ 2-Diabetikern den postprandialen Blutglukoseverlauf signifikant günstig zu beeinflussen, sofern eine kritische Menge an Nahrungsfasern (NF) pro 100 r g Kohlenhydrate (KH) überschritten wird. In der vorliegenden Studie wurde bei 18 Patienten mit Diabetes mellitus Typ 2 in randomisierter Reihenfolge der postprandiale Blutzuckerverlauf nach Einnahme von 'Spaghetti Napoli' mit 75 r g KH in Form von kommerziellen Vollkornspaghetti mit ausreichendem NF-Gehalt (7.7 r g/100 r gKH) bzw. konventionellen Eierspaghetti mit tiefer NF-Menge (2.7 r g/100 r gKH) verglichen. Durch Zugabe von Olivenöl und magerem Fleisch wurden die beiden Testmahlzeiten bezüglich Fett- und Proteingehalt angeglichen.Der Fasergehalt der Vollkornspaghetti (5.8 r g) bzw. Eierspaghetti (2.0 r g) differierte um 3.8 r g pro 75 r g KH. Hinsichtlich des Ausgangsblutzucker bestand keine signifikante Differenz (p=0.9661, Wilcoxon-Vorzeichen-Rangsummentest für gepaarte Analysen).Die Fläche unter der Kurve (AUC) nach Einnahme der Testmahlzeit betrug für Vollkornspaghetti 525.2 - 92.7 r mmol 2 Min m 1 2 l m 1 (m - SE) vs. 508.1 - 89.6 r mmol 2 Min m 1 2 l m 1 für Eierspaghetti. (p=0.9632, Wilcoxon-Vorzeichen-Rangsummentest für gepaarte Analysen).Parallel zur Blutzuckeranalyse wurden die C-Peptidkonzentrationen gemessen. Bei der statistischen Analyse zeigte sich hinsichtlich der Ausgangswerte erneut keine signifikante Differenz (p=0.5509, Wilcoxon-Vorzeichen-Rangsummentest für gepaarte Analysen).Die C-Peptidantwort (AUC) war für Vollkornspaghetti (43304 - 13679 r pmol 2 Min m 1 2 l m 1 ) tendenziell etwas tiefer als für Eierspaghetti (63361 - 16811 r pmol 2 Min m 1 2 l m 1 ).Die Differenz war statistisch allerdings nicht signifikant (p=0.4583, Wilcoxon-Vorzeichen-Rangsummentest für gepaarte Analysen).Der leicht erhöhte Faseranteil von kommerziellen Vollkorn-spaghetti bewirkt bei Patienten mit Diabetes mellitus Typ 2 im Vergleich zu traditionellen Eierspaghetti keinen günstigeren postprandialen Blutzuckerverlauf. Auch die C-Peptidantworten auf eine Stimulation mit den beiden Spaghettiarten unterscheiden sich nicht signifikant. Hauptgrund dürfte die insgesamt geringe Nahrungsfasermenge gewesen sein, obwohl die kritische Menge von 3 r gNF/100 r gKH überschritten wurde.Möglicherweise war die Qualität der Fasern jedoch ungenügend. In normal subjects as well as in Type-2 diabetic subjects increased fibre content of meals (exceeding 3 r g of fibre/100 r g of carbohydrates) has been shown to have a beneficial effects on the course of postprandial blood glucose levels.In the following study postprandial blood glucose levels following ingestion of test meals with two forms 'Spaghetti Napoli' with 75 r g of carbohydrates (whole-grain spaghetti with a fibre content of 7.7 r g/100 r g of carbohydrates vs. conventional egg-spaghetti with a fibre content of 2.7 r g/100 r g of carbohydrates) were compared in a randomised design in 18 patients with Type-2 diabetes mellitus. By adding olive oil and lean meat both meals were balanced with regard to fat and protein content, but the two test meals differed by 3.8 r g of fibre/75 r g of carbohydrates. Baseline blood glucose levels did not differ significantly (p=0.9661, Wilcoxon's rank-sum test for paired analysis). (p=0.9632, Wilcoxon's rank-sum test for paired analysis). In parallel, baseline C-peptide levels did not differ significantly (p=0.5509, Wilcoxon's rank-sum test for paired analysis). The C-peptide response (AUC) tended to be lower in whole-grain spaghetti (43304 - 13679 r pmol 2 Min m 1 2 l m 1 vs. 63361 - 16811 r pmol 2 Min m 1 2 l m 1 ).However, the difference was not statistically significant (p=0.4583, Wilcoxon's rank-sum test for paired analysis). Conclusion : The increased fibre content of so called whole-grain spaghetti compared to conventional spaghetti has no beneficial effect on postprandial blood glucose levels. In parallel, C-peptide levels do not differ significantly. This may be due to insufficient fibre content, although being above the critical level of 3 r g fiber/100 r g of carbohydrates. In addition, fiber quality of commercial whole-grain spaghetti is inadequate.
