Since the article was first produced, the authors have identified problems with the size and placement of the tables and figures. In order to rectify the deficiencies, the following changes have been made to the article.

  1. Figure 10 has been changed to a full-page, landscape-format graphic.

  2. Figure 13 has been changed to a full-page, landscape-format graphic.

  3. On page 11 figure 7 and Table 1 have been moved before Section 4.2.2. Root causes

  4. Figures 8 and 9 and Table 2 have been moved to come before Section 4.3

  5. Figure 15 and Table 4 have been moved before Section 5. Value of systems mapping.
